If the prize winner is from outside the state of Kerala, the documents to be submitted by the prize winner in person/by post for a prize ticket worth more than Rs. 1 lakh
The ticket must be submitted within 30 days from the date of the draw. (However, if the ticket cannot be submitted within the above-mentioned days, please submit it along with an explanation explaining the reason for the delay in submitting the ticket.)
Please submit the prize-winning ticket with the winner's name, address, pin code, and signature (as listed on the Aadhaar card) on the back.
After writing your name, full address, pin code (as mentioned in your Aadhaar card) and signature on the back of the ticket, have the photocopy notarized and submit it along with the name, job title, notary stamp and notary seal of the notary officer.
Fill in the application form available from the Lottery Sub-Office / District Lottery Office / Lottery Directorate / website click here and submit it, pasting two photographs of the winner, having the Notary Officer sign the photo, and recording the Notary Officer's name, job title, notary stamp and notary seal.
Fill in the receipt of the prize money, affix the signature of the winner on the revenue stamp, enter the full address and phone number of the winner, sign it with the signature of the notary officer, enter the name, job title, notary stamp and notary seal of the notary officer and submit it. Take a photocopy of both sides of the PAN card of the prize winner, get it signed by a notary officer, and submit it with the name, job title, notary stamp, and notary seal of the notary officer. Those who do not have a PAN card should submit it to the department along with the necessary documents within the ticket deadline. The amount will be disbursed on presentation of the PAN card

To know the account number and IFSC branch name of the winner, please take a photocopy of the page on which the account number and the bank passbook of the winner's single account are recorded, get it signed by a notary officer, and submit it with the name, job title, notary stamp, and notary seal of the notary officer. (Jan Dhan accounts and zero balance accounts are not accepted.)
The winner must submit a letter explaining the reason for coming to Kerala and the circumstances surrounding the ticket purchase/self-attested copy of any ID card issued by the Kerala Government.

The winner's signature, name and address (as on the Aadhaar card) should be written on the back of the prize-winning ticket in the specified space, a photocopy of both sides should be taken, self-attested and presented along with the prize-winning ticket.
